City & Hackney Referral form for GPs and other professionals:

To make a referral to City and Hackney Talking Therapies on behalf of your patient we require the following information. We accept referrals for those aged over 18 only and your patient must be registered with a GP in Hackney or live or work in the borough.
Please note:
We cannot accept referrals for patients with a Serious Mental Health illness that requires secondary care intervention.
We cannot accept those who are already on a waiting list for psychological therapy with a secondary care mental health service or PCPCS.
We are unable to accept referrals for patients who have a clinical dependency on alcohol/drugs.
